Defender Collins had made eight National League appearances for the Glovers this season, the last of which came in February.
A club statement read: “All at Yeovil Town Football Club are mourning the loss of club captain Lee Collins
“Lee sadly passed away yesterday and our thoughts and prayers go out to his family and friends.
“We ask everyone to respect the family's privacy at this time.

Collins started out at Wolves but did not make any appearances for the Molineux outfit before moves to Port Vale, Northampton, Mansfield and Forest Green Rovers.
He also helped Hereford United to promotion in 2007/08 and his move to Yeovil in 2019 was the first time he had dropped out of the Football League.
Collins arrived at Mansfield in June 2015, playing 72 games for the club over a two year period.
His final game for Yeovil was in a defeat at Stockport in February, where Collins was shown a late red card.
