The Connect service is targeting people who have lived independently but face increasing need due to age, mobility problems, disability, long-term health conditions or bereavement.

Nottinghamshire County Council is funding three organisations to run the services across the county:

• Metropolitan in Broxtowe, Gedling and Rushcliffe

• Age UK in Mansfield, Ashfield and Newark and Sherwood

• Nottingham Community Housing Association in Bassetlaw.

The services will encourage people at risk to manage their health, find solutions that help them to retain their independence and reduce loneliness and social isolation.

They will also look out for issues that will impact on their health and wellbeing, such as unsuitable housing or problems with managing finances. Older people are particularly at risk of loneliness along with people in poor health.

Councillor Muriel Weisz, chairman of the adult social care and health committee, said: “We are keen to reach out to Nottinghamshire residents with health problems or are isolated with this service to make sure they do not lose their independence or are impacted by loneliness.”

Residents concerned about someone in their community should contact:

• Bassetlaw: NCHA on 0115 844 3541

• Ashfield, Mansfield and Newark and Sherwood: Age UK on 01623 488217

• Broxtowe, Gedling and Rushcliffe: Metropolitan on 0115 939 5406.
