THE following Chad Sunday League clubs have not yest submitted their affiliation forms to the Notts FA and are required to do so immediately:
Warsop Swallows, AFC Bilsthorpe, Forest Tavern Annesley, Ling Forest, St Josephs, Sutton RS, Rainworth Robin Hood A and B, Kings Arms A, Sutton Pipelines United, AB Waste, And Why Not, Anvil and Acorn Signs, Mansfield Police, North Star, Countryman, Oak Tree Mango, Tam Talbot, Fox and Crown, The Red, Annesley Park Athletic, Cockle, Innovate, Matthews and Tannert.

Clubs still to send forms back to Mansfield District Council regarding grounds for next season, and should do so as soon as possible, are:

Bold Forester, Local Inn, Sandy Lane, Ritz Rangers, Lurcher, AB Waste, Rileys, Players Lounge Ladybrook, Ling Forest, Mansfield Police, PSV Woodhouse, Anvil and Acorn Signs, Victoria, North Star, AFC Welbeck, Oak Tree Mango, Nell Gwyn, Kings Arms, Smith Street.

The above-names clubs should forward their forms to the relevant authorities as soon as possible.
